Toolkit - Safe Administration of High-Risk IV Medications

The IV Safety Task Force created a "toolkit" that contains a list of standard concentrations and dosage units, and is intended to help other regions implement similar measures to standardize IV medication practices. It also includes practical advice on forming a regional task force, identifying variations in practice across different hospitals, developing standards and overcoming barriers that may arise during the process.
