PVP-I is a broad-spectrum, intermediate-acting, persistent antimicrobial solution utilized for surgical hand scrub, patient preoperative preparation and topical antiseptic. PVP-I's activity is based on its ability to be absorbed into the skin. The amount of molecular iodine present (free iodine) determines the level of antimicrobial activity. Available iodine refers to the total amount of iodine that can be titrated with sodium thiosulfate.

Typically 10% povidone-iodine formulations contain 1% available iodine and yield free iodine concentrations of 1 ppm. The antimicrobial activity of PVP-I can also be affected by pH, temperature, exposure time, concentration of total available iodine and amount and type of organic and inorganic compounds. PVP-I has bactericidal activity against gram-positive and gram-negative bacteria and is active against mycobacteria, viruses and fungi. Povidone-iodine has been classified by the FDA's Tentative Final Monograph (TFM) as a category I agent (safe and effective agent).
