"Jail Time for a Medication Error - Lessons Learned from a Pharmacy Compounding Error"

A pharmacist failed to detect a pharmacy technician's compounding error while preparing a chemotherapy infusion, and administration resulted in the tragic death of a 2-year-old patient. It also resulted in criminal charges for Eric, the pharmacist, who is now serving a 6-month jail sentence and has lost his pharmacy license permanently. This webcast will review the compounding error and hear details from the pharmacist's attorneys of the criminal prosecution. The ISMP staff will provide the Institute's perspective on compounding errors and check systems as well as practices that help avoid similar errors. The faculty will also discuss the second "victim" of an error—the practitioner who makes a fatal error—and potential future implications of criminal charges for medication errors.

 

Objectives:

  1. Review the anatomy of a compounding error that resulted in a patient's death.

  2. Identify practices that can lead to IV compounding errors and suggestions to prevent such errors.

  3. Discuss the future implications of criminal charges on transparency, accountability, and just culture.
